PARTICIPANT WAIVER

ATHLETIC PARTICIPANT ASSUMPTION OF RISK/ WAIVER OF LIABILITY/INDEMNIFICATION AGREEMENT

By agreeing to this form, I hereby certify that all of the information provided herein to Blessed Ballers Flag Football to register all of the participant(s) named is complete and accurate and that I am the legal parent or legal guardian of all of the participants listed. In consideration of being allowed to participate on behalf of the Blessed Ballers Flag Football program and related events and activities, the undersigned further acknowledges, appreciates, and agrees that: Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Blessed Ballers Flag Football, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law. I HEREBY FURTHER RELEASE, ABSOLVE, INDEMNIFY, AND HOLD HARMLESS BLESSED BALLERS FLAG FOOTBALL AND ALL OF ITS ELECTED AND APPOINTED OFFICIALS, EMPLOYEES, ACTIVITY OFFICIALS, AND ACTIVITY SUPERVISORS FROM ANY ACCIDENT, INJURY, OR DEATH THAT IS SUSTAINED BY ANY OF THE PARTICIPANT(S) NAMED BELOW DURING TRANSPORTATION TO OR FROM PAID ACTIVITIES, OR WHILE PARTICIPATING IN ANY PAID ACTIVITY, FROM ANY AND ALL LIABILITY.  I hereby agree to abide by, and to instruct the participants named below to abide by, all Blessed Ballers Flag Football program and facility rules, regulations, policies, and procedures, whether written or oral. I understand that Blessed Ballers Flag Football prohibits offensive conduct; discrimination on the basis of gender, race, color, national origin, religion, or disability; and physical, emotional, or sexual harassment by participants, parents, guardians, or friends during transportation to or from Blessed Ballers Flag Football activities and while participating in Blessed Ballers Flag Football activities. I understand that engaging in such behavior may result in the removal of participant(s), me, my family members, or my friends from all Blessed Ballers Flag Football programs.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
